You are eligible to participate in the employee benefits program if you normally work a minimum of 30 hours per week. If you enroll, you may also enroll your spouse and dependent children.
The benefits you elect begin on the first day of the month following 60 days of employment. Please make your elections within 30 days of employment.
The elections that you choose remain in effect until the next Open Enrollment period, unless you experience a qualifying life event.
